Requirements
All applicants must have the following:
- Valid Identification (Florida Driver’s License, Passport, etc.)
- Driver’s License
- RN / LPN / CNA License or HHA Certificate (minimum 75 hours training)
- HIV/AIDS Certificate (1-hour lifetime training)
- HIPAA Certification (for HHA)
- Assistance with Self-Administration of Medication Certification (if applicable)
- Current CPR / BLS / ACLS Certification
- CPR Card (if separate)
- OSHA Certificate (updated)
- Domestic Violence Certificate
- Level 2 Background Screening (completed, less than 5 years old or Live Scan)
- Signed Attestation & Affidavit of Compliance
- Two Professional References
- Proof of Liability Insurance (Professional Liability Insurance)
- Auto Insurance Card
- Auto Registration
- Proof of Residency or Citizenship (Voter Registration, Resident Card, etc.)
- Social Security Card / I-9 / W-9
- Payment Option (Direct Deposit / Voided Check)
- Physical Examination (within the last 6–12 months, stating free from communicable diseases)
